Upload COVID-19 Vaccination Record Card

All faculty and staff are required to receive a COVID-19 vaccination. Informing the college about your vaccination status is a requirement. This information is critical to our efforts to have complete information about what is happening on our campus with respect to COVID-19. Faculty and staff members must upload their proof of vaccination as soon as possible or upon full vaccination (full vaccination is 2 doses of the Pfizer or Moderna vaccine or 1 dose of the Johson & Johnson vaccine). In order to upload your card to this form, you must scan or take a picture of your card, email it to yourself and save it to your computer or Android phone.
If you prefer to submit a hard copy of your COVID-19 vaccination card, contact the Human Resources Office at firstname.lastname@example.org to make arrangements.
Why should faculty/staff submit proof of COVID vaccination?
A high vaccination rate ensures that campus life can return to near normal activity. Oberlin has an 85-90% vaccination goal. We request that everyone submit documentation so that we can ascertain the campus vaccination rate. Anyone who submits COVID-19 vaccination documentation AND is two weeks post-vaccine will be exempt from monthly PCR testing. If you are exposed to a positive person within 90 days after your vaccination, you will not have to quarantine, as recommended by the Center for Disease Control. If you are symptomatic of COVID-19 post-vaccination, contact your primary care provider for guidance.
While submission of vaccine documentation can help the college make decisions about increased in-person activities, remember that the COVID-19 Vaccination Record Card may also be required for personal activities, such as flying, avoiding quarantine at cities you visit or entering various venues. Put your card somewhere safe so that you can access it in the future. It’s your symbolic “passport to freedom”.
